Please contact Professor Sui-Yuan Chang (r434.syc.lab@gmail.com) to confirm details of services.
Step 2
Users will receive electronic files of “Service consultation form”, “Service application form”, and “Quotation”.
Step 3
Please complete and sign the “Service consultation form”, “Service application form” (in quadruplicate), and “Quotation” (in duplicate).
Step 4
- Make Payment.
- Please refer to “payment instructions” for payment procedures.
Step 5
Please mail the hard copies of “Proof of payment”, and signed “Service consultation form”, “Service application form” (in quadruplicate), and “Quotation” to BSL3 Research and Analysis Laboratory by registered mail.
Step 6
Once the payment is confirmed, we’ll schedule the service.
Step 7
A preliminary report (electronic file) will be offered to the User. After the User confirms the content of the preliminary report, the official report, payment receipt and service application form will be sent to the User.
- Once the payment is confirmed, we’ll schedule the service.
- No refund for any overpayment.
- Please do not pay in cash.
